L’infezione da Helicobacter R e l’autoimmunità portano, in una certa percentuale di casi, dopo una prima fase di gastrite superficiale, ad una gastrite atrofica con metaplasia intestinale

La meta­plasia intestinale consiste, invece, nella sostituzione delle cellule della mucosa gastrica con cellule di tipo intestinale

La presenza di gastrite atrofica e di metaplasia intestinale aumenta considerevolmente il rischio di carcinoma gastrico

Questo esame del Pepsinogeno è stato utilizzato come test di screening per identificare i soggetti affetti da gastrite atrofica e metaplasia intestinale, cioè quelli più a rischio di sviluppare un cancro gastrico; in questo modo è stato possibile selezionare quel gruppo di soggetti ad elevato rischio, da sottoporre a controllo endoscopico periodico, I primi risultati sono stati incoraggianti, portando nel gruppo di soggetti sottoposti a screening ad una riduzione della diagnosi del cancro gastrico avanzato con corrispondente aumento dei casi di diagnosi precoce che hanno permesso interventi terapeutici e chirurgici più efficaci e radicali
